Job description
The Chronic Disease Nurse Coordinator works closely with the Manager of Clinical Integration Primary Health Care. The Chronic Disease Nurse Coordinator is an expert clinical nurse who will be working closely with the programs within Clinical Integration Unit 1 (this includes specialties of Diabetes, COPD and asthma, Intermediate Care Services, etc). In collaboration with Primary Health Care (PHC) colleagues, the Chronic Disease Nurse Educator is responsible for providing clinical supervision of staff, orientation, certification, continuing education, assisting in the development of clinical pathways, and participating in development of policies and procedures relating to general and specialty chronic conditions care. The incumbent will participate in program planning and evaluation. Primary responsibilities include ensuring services follow the Diabetes Canada Clinical Practice Guidelines, Canadian Thoracic Society Guidelines, etc.
Experience
- Minimum 1 year supervisory experience
- five (5) years' recent nursing experience in Diabetes and Respiratory education
Education
- Bachelor of Science in Nursing (BScN)
Memberships
- College of Registered Nurses of Saskatchewan (CRNS)
