Temporary Personal Plan Facilitator

TEMPORARY FULL-TIME PERSONAL PLAN FACILITATOR-Waldheim

40 hours per week, working day shifts. Applications will only be accepted through the web link.

The Personal Plan Facilitator works with the person using services, their circle of support (including the Karis Disability Services team(s) that provides support) to learn about the person and use this information to facilitate the creation of a comprehensive Personal Plan. The plan will address the persons needs, hopes and dreams with an eye toward building communities of belonging in accordance with the core Values of Karis Disability Services.

Requirements

• A Valid Sask. Full Class 5 drivers license and access to reliable transportation as travel may be required,
• Satisfactory Drivers Abstract
• Satisfactory Criminal Record Check with Vulnerable Sector Check
