Referral Navigator - Coordinated Access Regina

• Provide outreach, housing navigation and coordinate referrals for individuals and families who are experiencing, or are at risk of experiencing, homelessness
• Connect and collaborate with an extensive community network
• Monitor the By-Name List; proactively engage with individuals who are not yet connected to CAR and support them through the referral process
• Assess participant readiness and referral requirements e.g, consent, eligibility, documentation, etc.
• Match and refer individuals to CAR housing vacancies; facilitate warm hand-offs to receiving agencies; assist with accessing services
• Maintain confidential and accurate HIFIS records
• Provide trauma-informed, culturally-responsive, person-centered, and harm-reduction support
• Conduct and coordinate encampment outreach in collaboration with others
• Track and report on encampment needs, activities, contacts, and data to support a coordinated plan
• Active participation in meetings e.g., CAR partner, encampment, PATH, extreme weather and other meetings
• Communicate gaps, needs, and barriers to housing/service access

Other requirements

experience working in outreach, homelessness, housing, mental health, or related field; understanding of community programs/resources; communication, teamwork, interpersonal, crisis intervention, conflict resolution, problem-solving skills; valid driver's and access to a reliable vehicle; submit/maintain a successful criminal record check and vulnerable sector check
