GENERAL ACCOUNTABILITY
Nurse Navigator is responsible for the coordination, development, implementation, administration, evaluation, and delivery of patient navigation. The navigator will work collaboratively with patients to ensure individualized patient centred care is achieved in the timeliest manner possible. In collaboration with health system partners and clinicians, the Navigator acts as a clinical resource and liaison providing support, leadership, direction, and education to facilitate the coordination and continuity of services in matters related to the patient. This position reports to the Provincial Manager, Systemic Therapy and Outpatient Services.
QUALIFICATIONS
Education
- Bachelors degree or Diploma in Nursing
- Completion of the Oncology nursing certification CON(C) or equivalent. Where this is not complete the successful candidate is required to obtain certification within two years of date of hire to the position.
Experience
- Five (5) years nursing experience.
- Demonstrated experience in leadership and/or program development and evaluation and/or patient education is required.
- Minimum 3 years of Disease Site and/or Navigator and/or Educator and/or Oncology nursing experience is required.
