Job description
The Social Worker - BSW Senior, reporting to the Manager or their designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals, and consultation to appropriate services and provides on-site and virtual social work services.

- 2 years SW experience working with issues of capacity, dementia, grief, and loss

- Ability to work in a team setting in a postive manner
- Knowledge in medical terminology
