Outreach Worker

PARTNERS Family Service is a community-focused non-profit organization dedicated to supporting individuals and families, strengthening community connections, and improving access to essential resources and services.

Position Overview

We are seeking a compassionate, motivated, and community-minded Outreach Worker to join our team. The successful candidate will engage with individuals and families in the community, identify their needs, provide support and referrals, and connect them with available programs, services, and resources.

Key Responsibilities

Build positive, trusting relationships with individuals, families, and community members.

Conduct community outreach to identify needs and connect people with appropriate services.

Provide information, support, advocacy, and referrals to community resources.

Assist clients in accessing housing, food assistance, mental health supports, employment services, and other essential resources, as applicable.

Support the planning and delivery of community programs, workshops, and events.

Collaborate with local agencies, service providers, and community partners.

Maintain accurate, confidential client records and prepare reports as required.

Qualifications and Skills

Experience in community outreach, social services, community development, or a related field is an asset.

Education in social work, human services, community development, or a related discipline is preferred; equivalent lived or professional experience may be considered.
