Job Description: Reviews, develops and evaluates educational material. Assists in program planning, implementation and evaluation of activities. Maintains accurate client records, statistics and workload measurement tools. Provides health information, counseling, and referral through telephone and one on one contacts in a clinical, home and community setting. Assesses for continuing risk and immunization/prophylaxis of at-risk contacts. Provides individual client and group education programs related to program objectives. Assists in the investigation and control of illness outbreaks including case identification and implementation of control measures, including in care facilities. Uses strategies to reduce communicable disease such as surveillance, immunization, early case identification, timely follow-up and prophylaxis/treatment. Consults with health care professionals regarding communicable diseases. Provides observational experiences for medical students and precepts nursing students.

Experience
- Minimum of one (1) year Public Health Nursing experience is an asset.

- Bachelor of Science in Nursing (BScN)
