Job Description: Assisting with developing, coordinating and conducting inpatient and outpatient programs and clinics; using established criteria to assess and plan each client's self-management care needs, providing intervention ensuring timely access to care; providing efficient and effective individual self-management education/counseling for patients/families/caretakers in all aspect of management care and treatment. This includes liaising with families, physicians, dietitians, staff nurses and community agencies.
