About this role
The CA Intake and Assessment Coordinator will work together with the CA Supervisor and the CA team to provide intakes and assessments for people experiencing housing insecurities. This role works directly with the vulnerable populations and requires an understanding of housing insecurities and homelessness in Saskatchewan.
Hours of work The position may require flexible working hours, including evenings and weekends, as needed.
Key Responsibilities
Conduct intakes, assessments and referrals for people accessing Coordinated Access.
Assess and process prospective clients using program assessment measures, respecting clients dignity, and confidentiality.
Monitor, manage and report intake and assessment status.
Collaborate with Reaching Home subprojects to ensure people requiring homes are referred to the appropriate organization and housing option.
Empower clients by educating them about their rights and responsibilities.
Manage the By-Names List through a data-management system.
Develop and maintain an inventory list of available housing options.
Develop effective and collaborative working relationships and partnerships with community agencies and stakeholders.
Stay current with evolving practices and emerging issues that have the potential to affect clients experiencing housing insecurities and homelessness.
As a housing team member, contribute to the creation and continued improvement of data collection processes and strategies.
Contribute to the creation and continued improvement of strategies and policies to advance the goals of Coordinated Access.
Input, monitor, manage, and report intake and assessment status.
Other duties as assigned by supervisor or SHIP management
