In this role, you will maintain an assigned caseload and provide clinical leadership, guidance, and resources to support the unit. You will play an important role in developing and coordinating individualized supports that respond to the unique needs, strengths, and goals of the individuals we serve.
Duties and Responsibilities
Services
- Develop individual and family treatment plans for participants in collaboration with internal and external stakeholders.
- Provide clinical oversight and lead the implementation of individual and family treatment plans including all aspects of participation in the treatment program, completion of documentation, and providing information and feedback to the stakeholders.
- Conduct clinical assessments and refer for specialized services (psychological assessments, counselling, clinical resource needs, adjunct therapies, and psychiatric services) when or if deemed necessary. Communicate with referral agencies and attend clinical appointments with participants when required.
- Conduct individual, group, and family counselling.
- Advocate on behalf of the participants.
- Lead planning conferences, family consultations, and conferences with health care professionals. Participate in educational conferences and conferences with other stakeholders as required.
- Communicate and collaborate with internal and external stakeholders including families and referring agencies.
- Act as a liaison between the group living and education programs regarding the participants service plan.

$72,987.20 - $111,404.80
