Street Culture Project is hiring a full-time Case Plan Coordinator to oversee intake, case management, and transition planning for youth in our Transitional Housing programs.
Location: 1314 11th Avenue, Regina, SK
Hours: MondayFriday, 8:30 a.m.5:00 p.m.
Wage: $21.50$24.00/hour
Start: Immediately upon selection.
Responsibilities
- Coordinate referrals, intake, discharge, and youth transitions.
- Develop individualized case plans within required timelines, collaborating with youth, MSS, healthcare providers, schools, and other supports.
- Advocate for youth and connect them with community resources, health services, and educational opportunities.
- Maintain accurate, timely records in Apricot, including case notes, safety plans, and medication administration records.
- Develop youth-specific safety plans and provide support during crises.
Qualifications
- At least two years of experience in youth care or case management.
- Experience in case planning, advocacy, or resource coordination preferred.
- Strong communication, documentation, organization, and time-management skills.
- Understanding of trauma-informed care and harm reduction.
- Ability to work independently and collaboratively and remain calm under pressure.
- Secondary school completion required; related post-secondary education preferred.
Requirements
- Valid Saskatchewan drivers licence.
- Criminal Record Check with Vulnerable Sector Clearance.
- Commitment to SCPs values and approaches.
- CPR/First Aid, ASIST, Crisis Intervention, or Mental Health First Aid training is an asset.
